To use vertebrate animals in the laboratory or in the
field requires approval from the university's Institutional Animal Care
and Use Committee (IACUC).
Approval is based on review of an animal use protocol and typically
requires 3-4 weeks. All animal users must also submit a Risk Assessment
to the office of Environmental Health and Safety (EH&S) and must take an online training course ("Working
with the UF IACUC).
It would be a major violation of federal regulations to keep any
vertebrate animal anywhere on campus for any length of time without
IACUC approval.

•This form will download to Adobe Acrobat Reader. Acrobat Reader can
be obtained for free from Adobe's
web site. THIS FORM CAN BE FILLED OUT IN ACROBAT READER.
'Click' on the blanks to fill in the blanks - then print it out or send it
electronically. This form does not go to IACUC. Its purpose is for assignment
of animal room space in Batram and Carr Halls. Please submit it to Zoology's
IACUC representative (currently, Doug Levey).
